Sacred Heart Mission's Journey to Social Inclusion (J2SI) program is an evidence-based intervention designed to break the cycle of chronic homelessness. We know the current service system is not effective and we're taking a different approach in our goal to end homelessness.
We're now hiring for our third and final team of this ground-breaking initiative, and you'll have a direct hand in its success. Join a brand-new team of experienced staff with diverse professional backgrounds. Over a three-year journey, we will be delivering cutting-edge interventions with proven outcomes in stable housing, social inclusion and economic participation.
Your new role
The Intensive Case Manager is a full-time role designed to foster the independence of our participants and support them to achieve the life they want to live. We are looking for dynamic and motivated team players who have:
- Relevant degree or above qualifications
- Experience working with participants with complex needs
- A strength-based approach
- Resilience, flexibility and adaptability
- Creative outcome-focused attitude

Your day-to-day
While every day is different, a typical day might see you on outreach, visiting one of your many participants and linking them to appropriate services. You may then return to the office for a check-in or group reflective practice with your colleagues.
For the rest of the day, you will likely be:
- Utilising our Case Management Framework to progress participant goals and ensure strong service delivery outcomes
- Working with participants to develop support plans that are holistic and contain clear, specific strategies for sustainable change
- Providing assertive and planned outreach to participants in their preferred environment
- Developing safety assessments with risk management strategies
- Service coordination with other agencies or partners

- 2-week program-specific orientation, starting 20 July 2020
- Structurally supportive work environment: regular line manager direct supervision, 1-on-1 clinical supervision, in-house training dept, group reflective practice, team days, access to an employee assistance program
