Your New Role
Homefront is a program of Sacred Heart Mission's Women’s Services that provides crisis accommodation and is committed to delivering a gender specific response to the needs of women experiencing homelessness and other associated issues. to be responsible for providing the information, support, and case management to women who access the program. This is a 12 month full time contract.
The purpose of this role is to ensure that Homefront provides the highest quality, most effective and caring service to women in accordance with SHM’s values and legislative and funding requirements. Also, to ensure that Homefront responds flexibly to the immediate needs of women who are experiencing homelessness, or at imminent risk of homelessness and/or in other crises, through the provision of information, support, and case management to find sustainable housing options.
Your Day-to-Day
As a Case Manager, you will:
- Provide person-centred support and case management to women residing in Homefront crisis accommodation, assisting them to build safety, stability and independence while preparing for and transitioning to sustainable housing outcomes.
- Deliver flexible case management and outreach support to women residing in crisis accommodation, Transitional Housing Management (THM) properties and long-term housing, supporting tenancy establishment, sustainment and successful community settlement.
- Respond to the day-to-day operations of the Homefront site, providing timely client support, managing competing priorities, and contributing to a safe, welcoming and well-functioning environment.
- Work collaboratively with colleagues and partner agencies through effective teamwork, communication, case coordination and handovers to ensure coordinated and client-centred support.
