Your new role

We have an incredible full-time opportunity for a Case Manager - Sacred Heart Central to join our dynamic and collaborative team.

Come join our team who are dedicated to ending homelessness and work towards that by focusing on clients in a person-centered outreach capacity across the inner metro area. 

No two days are the same in this role; balancing assertive engagement with case management, all focused on supporting women into permanent housing!

Your day-to-day

A typical day in this role involves managing a caseload in an outreach capacity, working closely with fellow team members, internal programs and external agencies. You’ll coordinate and monitor caseloads, help clients achieve their goals, and empower them to secure and maintain long-term housing stability. Each interaction you have will be focused on supporting clients to build sustainable, life-changing housing outcomes.

Essentials for success

  • A degree in Social Work, Social Sciences, Psychology or relevant field. 
  • Demonstrated experience in case management and navigating service systems. 
  • Ability to adapt to clients' needs effectively and include the voice of the client while collaborating with different services.
  • Passionate about providing trauma-informed care practices.
  • Skills in managing challenging behaviours and can stay calm and effective in crisis situations.
  • Experienced in the supportive housing / women and homelessness space (desirable, other community service areas considered).

  • Social and Community Services Employee Level 4 ($45.93 - $49.42) (Pay point dependent on qualifications and experience).
