Intensive Case Manager - J2SI (Multiple Roles)

Your New Role

The Journey to Social Inclusion Program (J2SI) is working against chronic homelessness with its innovative housing-first approach, combined with comprehensive, personalised support. We’re thrilled that J2SI has secured new funding, ensuring staff for 3 years to expand a 6th phase across Victoria with this groundbreaking program.

As an Intensive Case Manager for J2SI you will be focused on building strong relationships with participants who have experienced long-term homelessness. You will work with a small caseload allowing for individualised goal planning and regular face-to-face appointments, while focusing on preventative support and strategies that help break the cycle of homelessness, ensuring participants receive the care and resources they need to move forward. 

We have multiple maximum-term full-time or part-time positions (Monday-Friday) available to start 17th February 2025.

Your Day-To-Day

Each day brings new opportunities and challenges to make a difference. The Intensive Case Manager role supports people not just in obtaining housing, but also in moving in and sustaining their new homes, ensuring long-term stability. A big part of this role is to facilitate connections to community resources and services, ensuring participants have the support they need to thrive. You'll have the chance to work with the same participants throughout the entire program, fostering stability and creating opportunities for meaningful change 

 Essentials for Success

  • Experience in case management and navigating complex service systems.
  • Skills in managing challenging behaviours and can stay calm and effective in crisis situations.
  • Understanding of and have respect for cultural diversity and can practice cultural safety. 
  • Passionate about providing trauma-informed care practices.
  • Ability to adapt to participants' needs effectively and include the voice of the participant while collaborating with different services. 

 You Can Look Forward To

  • Becoming a part of an evidence-based program that is at the forefront of targeting chronic homelessness.
  • $44.37-$47.75 per hour (depending on qualification and experience). 
  • Salary packaging expenses like your rent, holiday accommodation and dining out: up to $15,990 pre-tax.
  • Contracts ranging from 2-3-years ending March 2027 or 2028. 
  • 2-week program-specific orientation, starting 17 February 2025.   
  • A supportive work environment: regular line manager direct supervision, clinical group reflective practice, Psychological First Aid, EAP, Community of Practice spaces.
